The second step is to email the following documents to mainoffice@pacek-8.org

Birth Certificate
Immunization Record
Health Appraisal with Vision Screening (Kindergarten Only)
Current Report Card (1st - 8th grades)
Current Individual Education Plan - IEP if the student has Special Education Services
Discipline Report from the previous school
Parent ID

After we receive the documents we will contact you to schedule the Parent and Student interview.
